Community Services


Adult Day Program

Objectives

· To provide structured and supervised activities for post-stroke and dementia patients and frail seniors in order to prevent social isolation and encourage independent living in the community
· To offer social, recreational, physical and educational activities in order to maintain the optimal level of functioning and delay institutionalization
· To provide respite, support and information services for caregivers

Eligibility

· Persons who require assistance in daily living
· Persons who require health monitoring and supervision on a regular basis
· Persons with cognitive impairment such as Alzheimer’s disease
· Persons who are isolated from the community due to functional limitations
· Persons who are at risk of being left alone
· Persons whose caregivers require regular periods of relief

Program

· Educational, physical, recreational and social activities
· Assistance with activities of daily living
· Individual/group supportive counselling
· Individual/group physiotherapy and occupational therapy sessions
· Snacks and hot lunch
· Foot care service
· Caregiver support program

Time

Integrated Day Program Special Day Program Markham Day Program Mississauga Day Program
Monday to Friday Monday to Friday Tuesday to Friday Monday to Friday
9:30 a.m. to 3:00 p.m.*

* Extended hours available (8:30 a.m. – 5:30 p.m.) at the Mississauga Centre; additional fee for extended hours is $8 per person each day.

Fees

· $20 per day for the Scarborough and Mississauga Day Programs; $22 per day for the Markham Day Program. Fees include all activity expenses
· Additional fees will be charged for transportation, outings and special events

Application

· Referrals are welcome from medical professionals, social workers, family members, friends or clients themselves
· Personal interview is required to determine eligibility
· Application for the Mississauga Day Program should go through the Community Care Access Centres
