Chronic Disease Self-Management Program
Disease Specific Program - Living Well with Chronic Pain
This program is designed for persons who are experiencing chronic pain. It is identical to the evidence-based Stanford model of Chronic Pain
Self-Management Program comprised of six weekly sessions of 2.5 hours each:
Session 1
- Introduction - Identifying common feelings and concerns
- Debunking myths about chronic pain
- Differences between acute and chronic pain
- Introduction to the pain and symptom cycle
- Introduction to action plans
Session 2
- Introduction to physical activity and exercise
- Pacing: Balancing activity and rest
- Better breathing and progressive muscle relaxation
- Making an action plan
Session 3
- Moving Easy Program
- Dealing with difficult emotions
- Using the mind: Distraction
- Fatigue and sleep management
- Guided imagery
- Making an action plan
Session 4
- Moving Easy Program
- Communication skills
- Healthy eating
- Problem solving
- Making an action plan
Session 5

- Medication and chronic pain
- Dealing with the blues and depression
- Positive thinking

Fee
$50 for six sessions per person.
