Chronic Disease Self-Management Program
Disease Specific Program - Living Well with Diabetes
Three disease specific programs are offered. Living Well with Diabetes and Living Well with Parkinson’s Disease
each comprises six sessions of Stanford CDSMP program plus four disease specific sessions focussing on education and supervised exercise.
The Living Well with Chronic Pain program comprises six sessions.

- Healthy eating (by dietician)
- Tips for dining out for patients (by dietician)
- Outing – lunch (by group leader)
- Action planning
Session 9
- Medication and diabetes (by pharmacist)
- Manage diabetes by better blood sugar and insulin control (by pharmacist)
- Action planning (by group leader)
Session 10
- Foot & eye care of diabetes (by foot & eye care professionals)
- Emotional aspect of diabetes (by social worker)

Fee
$100 for ten sessions per person. Participants are welcome to come with one family member at no extra cost.
