Community Services


Objectives

Yee Hong provides a full spectrum of community-based services to enable seniors to maintain their independence and dignity and to live their lives to the fullest. Different programs and services are developed to promote social integration and quality of life and counter the sense of isolation and loneliness common among seniors.

Philosophy

Similar to the other services of Yee Hong, community-based services operate on the principles of client-centred and holistic approach to care. The core values of respect for client's wishes, acceptance and self determination are deeply embedded in the fabric of service provision. As well, we take a holistic approach that cares for the physical, emotional, social and spiritual well-being of seniors.

Funding

Most of the community-based programs are funded by the Central East LHIN, Central LHIN and Mississauga Halton LHIN. Prior to providing funding for the provision of services to its local health system, the Local Health System Integration Act, 2006 requires that the LHIN and the Health Service Provider (Yee Hong) enter into a service accountability agreement.

The service accountability agreement is a multi-year agreement. It supports a collaborative relationship between the LHIN and the HSP to improve the health of Ontarians through better access to high quality health services, to coordinate health care in local health systems and to manage the health system at the local level effectively and efficiently.
