Respect for Privacy and the Right to Privacy
The Yee Hong Centre respects the right to privacy of all individuals from whom
personal and personal health information is collected. This extends to individuals who
seek and receive services from this organization and to those who work for and
work with this organization in the direct and indirect delivery of services.
Collection, Use, and Disclosure of Personal Health Information
The Yee Hong Centre Privacy Policy references Ontario’s Personal Health
Information Protection Act (PHIPA) and Quality Care Information Protection Act (QCIPA).
Personal/personal health information is collected and used by the Yee Hong Centre as
permitted or required by law, and with informed consent of the individual or
substitute decision maker as applicable, primarily for the purposes of determining
need for services, planning services, and accessing related services.
Ten internationally accepted principles
provide the basis upon which the privacy policy of the Yee Hong Centre has been developed.
In-keeping with the PHIPA, the Yee Hong
Centre and Yee Hong Community Wellness
Foundation share a contact information
database for fundraising purposes.
Individuals wishing to withhold/withdraw
consent for the collection, use, and
disclosure of personal/personal health
information should inform the Yee Hong
Centre, and/or the Foundation.

confidentiality of personal/personal health
information in the possession of the Yee
Hong Centre as a Health Information

The Yee Hong Corporate Privacy Officer, can

available to facilitate resolution of
privacy issues that cannot be resolved
directly with other individuals affiliated
with the Yee Hong organization.
