How does one apply for a long-term care bed at one of the four Yee Hong Centres?
Admissions to Yee Hong’s long-term care centres are coordinated by the Community Care Access Centres (CCAC).
Applicants may apply for beds in the basic wards (57%) or private rooms (43%). There are limited short stay beds
at the four centres. To apply, please contact the Placement Coordination Service of your local CCAC.

Does Yee Hong provide services and programs outside the four centres?
Yes. Yee Hong reaches out to different communities with off-site service centres or offices in York Region and North York. Programs offered outside our four centres
include Congregate Dining, Friendly Visiting,
Client Intervention and Assistance, Active Senior and Outreach Program, and Care Ambassador Program.

Does Yee Hong provide any service for Alzheimer's patients and stroke survivors?
There are Alzheimer's residential care units at all four Yee Hong Centres. We also provide Special Adult Day Programs for Alzheimer's and
post-stroke clients in the community. Yee Hong runs the first and only Chinese program in the GTA for younger stroke survivors under the age of 65.

How does one access the medical services affiliated with Yee Hong?
A range of medical clinics from family medicine to different medical specialities are available and services vary at the following
Yee Hong Centres. Referral by a physician is required for specialist consultations. Please call the centres to make an appointment.

How does one apply for Yee Hong Aw Chan Kam Chee Evergreen Manor?
Two types of units are available at Yee Hong Aw Chan Kam Chee Evergreen Manor: rent-geared-to-income units and market rent units.
Application for rent-geared-to-income housing should be made through Housing Connections (Tel: 416-981-6111). Application for the market
rent units should be directed to the Management Office of Yee Hong Aw Chan Kam Chee Evergreen Manor at 416-298-0688.
