Memory Program
This is the first and only memory program offering Chinese and English cognitive
tests in the GTA. Dr. Gabriel Chan, geriatrician and founder of the program, and Dr. Joyce Lee test seniors
with symptoms of memory decline and provide treatment and care recommendations for referring
physicians, patients and families. Seniors who require assistance will be referred to Yee Hong’s Client
Intervention and Assistance Services.
Consultations are provided in response to physician referrals only. For inquiries and appointments, please call 416-321-6679.
Cancer and Palliative Care Services
Since 1998, the Yee Hong Centre has been delivering in-home palliative care services to residents in north Toronto and southern York Region.
As of December 1, 2007, this program is no longer accepting new referrals. All clients already in the program will be provided with the full
range of services. New patients and their families will be helped to connect with other palliative care and hospice resources.
This program had originally been launched with the anticipation that government support could be secured to sustain operations. As there is no
indication that government funding for the existing model of service delivery will become available in the foreseeable future, and the required
amount of physician resources to deliver services at the intended level of effectiveness cannot be assured, the difficult decision has been
made to wind down the Yee Hong in-home palliative care services at this time.
Yee Hong is still committed to meeting community need for end-of-life care, and is actively working with the Central East Local Health
Integration Network and other stakeholders to explore other means of providing sustainable palliative care services to the community,
including the possibility of establishing a residential hospice.
Further information about this new development will be announced when specific information becomes available.
