Yee Hong Centre for Geriatric Care
Privacy Policy
At Yee Hong Centre for Geriatric Care,
protecting your privacy is very important to
us. Our goal is to treat the personal health
information that you provided to us with
utmost respect and confidentiality. We are
committed to a high standard of privacy for
our information practices. As its Privacy
Policy, the Centre has adopted the 10
international recognized privacy principles
known as the fair information practices.
This Privacy Policy was last revised on July
8, 2004.
Principle 1:
Accountability for personal

Yee Hong Centre for
Geriatric Care is responsible for
all personal information under its
custody or control, including
information which it may transfer to
third parties for processing. The
Centre develops several
privacy-related policies, including
establishing procedures to receive
and respond to complaints and
inquiries; training and
communicating to staff and
volunteers about the Centres privacy
policies and practices, and
developing publicly available
information to explain our privacy
policies and practices. The Centre
has a Privacy Steering Committee and
has designated the Privacy Officer
who is accountable for compliance
with the following privacy
principles.

Principle 2:
Identifying purposes for the
collection of personal information

Yee Hong Centre for
Geriatric Care will identify (if
necessary, explain) the purposes for
the collection prior to or at the
time of collection. The Centre
collects personal information for
the following purposes: deliver
care, for administration, in
teaching, research, statistics,
fundraising, and to comply with
legal and regulatory requirements.
Unless a new purpose is legally
required, consent must be obtained
before the information can be
collected, used, disclosed and
retained.

Consent for the collection, use, and
disclosure of personal information

The knowledge and
consent of the individual are
required for the collection, use or
disclosure of personal information,
except where inappropriate, for
example, legal, medical, or security
reasons may make it impossible or
impractical to seek consent. The
Centre does not share personal
information beyond the use that it
was intended. Consent may be
withdrawn at any time, subject to
legal or contractual restrictions
and reasonable notice.

Limiting collection of personal

The collection of
personal information will be limited
to that which is necessary for the
purposes identified by Yee Hong
Centre for Geriatric Care.
Information will be collected by
fair and lawful means. |

Limiting use, disclosure and
retention of personal information

Personal information
will not be used or disclosed for
purposes other than those for which
it was collected, except with your
consent or as required by law.
Personal information will be
retained only as long as necessary
for the fulfillment of those
purposes, and destroyed in
accordance with legislation,
hospital policies, guidelines and
procedures.

Ensuring accuracy of personal
information

Yee Hong Centre for
Geriatric Care will take reasonable
efforts to ensure Individuals
personal information is accurate,
complete and up-to-date. Individuals
may request amendments to their
personal information.

Principle 7:
Ensuring safeguards for personal
information
|
Yee Hong Centre for
Geriatric Care applies security
safeguards appropriate to the
sensitivity of the information to
aim to protect against loss, theft,
unauthorized access, disclosure,
copying, use, or modification,
regardless of its format. Protection
may include physical measures (i.e.
locked filing cabinets and
restricted access), organizational
measures (limiting access on a
need-to-know basis), and
technological measures (use of
passwords, encryption and audits).
All staff and agents are required to
sign a confidentiality agreement as
a condition of employment, or
contract.

Principle 8:
Openness about personal information
policies and practices
|
Yee Hong Centre for
Geriatric Care makes specific
information about its privacy
policies and practices readily
available to the public. This
information will be easy to
understand and be made available in
multiple formats such as brochures
and website.

Principle 9:
Individual access to their own
personal information
|
Upon request, within
a reasonable time and at a
reasonable cost, individuals will be
informed of the existence of their
personal information and will be
given access to it. They can
challenge its accuracy and
completeness and have it amended as
appropriate. The Centre may deny or
restrict their access in limited
circumstances, with reasons provided
upon request. This may include
information that is prohibitively
costly to provide; refers to other
individuals; cannot be disclosed for
legal, security, or proprietary
reasons, or is subject to
solicitor-client or litigation
privilege.
In order to safeguard personal
information, you must provide
sufficient identification
information to permit the Centre to
identify the existence of personal
information, including details of
third-party recipients.

Principle 10:
Challenging compliance with Yee Hong
Centre for Geriatric Cares policies
and practices
|
Yee Hong Centre for
Geriatric Care has procedures for
addressing and responding to all
inquiries or complaints about its
management of personal information.
It will investigate all complaints.
If a complaint is justified, it will
take appropriate measures,
including, if necessary, amending
its policies and procedures.
Individuals wanting to address a
challenge concerning Yee Hong Centre
of Geriatric Cares compliance with
the above privacy principles are
encouraged to contact the department
manager who will explain Yee Hong
Centre of Geriatric Cares privacy
complaint process and start the
investigation. Yee Hong Centre of
Geriatric Care will ensure that the
concerns are investigated promptly
and a response is provided in a
timely manner.
