How do I
register?
Please collect
funds from your
sponsors and
complete the
registration
form.
Submit the
completed form
at Spring
Into Wellness
(June 7 & 8 – Mississauga or June 14 – Scarborough-Finch).
To be eligible
for incentive
prizes, all
monies must be
submitted at the
event. To be eligible for the top fundraiser prizes,
all monies must be received at the Foundation Office
(Scarborough-Finch, 416-321-0777) no later than June 23.
Please photocopy additional pledge forms as required.
