Best Practice Spotlight Organization (BPSO)

Advancing Evidence-Based Care at Yee Hong

Yee Hong Centre for Geriatric Care is proud to participate in the Best Practice Spotlight Organization® (BPSO®) program, an initiative of the Registered Nurses’ Association of Ontario (RNAO).

Through this partnership, Yee Hong is strengthening its commitment to evidence-based practice, continuous quality improvement, and person- and family-centred care across its long-term care communities.

As a BPSO pre-designate organization for the 2026–2029 period, Yee Hong is committing to undertake a collaborative journey to implement, evaluate, and sustain RNAO Best Practice Guidelines (BPGs), supporting consistent, high-quality care for the residents and families we serve.

About the BPSO Program

The BPSO Program is an internationally recognized initiative led by the RNAO.

The program supports healthcare and academic organizations in systematically implementing evidence-based Best Practice Guidelines to enhance professional practice, strengthen organizational cultures of quality, and improve health outcomes.

Organizations participating in the BPSO pre-designation program undertake a three-year journey focused on implementing selected guidelines, developing staff capacity, evaluating practice changes, and building systems to sustain improvements.

The BPSO program emphasizes collaboration, leadership, knowledge exchange, and the meaningful involvement of health-care professionals, residents, families, and organizational partners.

Our Commitment at Yee Hong

Yee Hong’s participation in the BPSO program reflects our ongoing commitment to delivering compassionate, culturally responsive, person- and family-centred care.

Building on Yee Hong’s established care philosophy and organizational values, the BPSO initiative provides a structured approach to translating evidence into everyday practice.

Through this journey, we aim to:

  • Strengthen the integration of evidence-based practices into resident care and organizational processes.
  • Promote safe, high-quality, person- and family-centred care.
  • Foster interdisciplinary collaboration, professional development, and shared leadership.
  • Empower frontline staff to lead and sustain meaningful practice improvements.
  • Support consistency and continuous quality improvement across participating Yee Hong communities.
  • Evaluate the impact of practice changes and share knowledge with the broader health-care community.

Our BPSO journey is a collaborative effort involving corporate leadership, site leadership, registered nurses, registered practical nurses, personal support workers, allied health professionals, and other interdisciplinary team members.

Together, we are building a culture in which evidence, compassion, and continuous learning guide the care we provide.

Our Best Practice Guidelines

Yee Hong is focusing on the implementation of three RNAO Best Practice Guidelines as part of its BPSO pre-designation journey.

1. Person- and Family-Centred Care

Person- and family-centred care recognizes residents and families as essential partners in care.

This approach emphasizes understanding each person’s individual needs, preferences, values, cultural background, and goals while supporting meaningful participation in care planning and decision-making.

Through this guideline, Yee Hong aims to further strengthen respectful communication, collaborative decision-making, and care experiences that reflect what matters most to residents and their families.

2. Preventing Falls and Reducing Injury from Falls

Falls prevention is essential to promoting resident safety, mobility, independence, and well-being.

This guideline supports evidence-based approaches to identifying fall risks, implementing individualized prevention strategies, and reducing the likelihood and consequences of fall-related injuries.

Through interdisciplinary collaboration, resident assessment, education, and continuous evaluation, Yee Hong aims to strengthen falls-prevention practices and promote safer care environments.

3. Transitions in Care and Services

Safe and coordinated transitions in care are essential to continuity, quality, and positive resident experiences.

This guideline focuses on improving communication, collaboration, planning, and information-sharing when individuals transition between care providers, services, or health-care settings.

At Yee Hong, implementation will focus on strengthening interdisciplinary coordination, engaging residents and families, and supporting more seamless transitions throughout the continuum of care.

Site BPSO Co-Leads and Implementation Teams

Each participating Yee Hong community is supported by designated BPSO Co-Leads and interdisciplinary implementation teams.

These teams work collaboratively with corporate leadership to coordinate guideline implementation, support staff education, engage Best Practice Champions, monitor progress, and sustain evidence-based practice improvements.

The BPSO governance model supports both organization-wide consistency and the ability to adapt implementation strategies to the needs of individual communities.

Participating Yee Hong Communities

Yee Hong’s BPSO initiative is coordinated centrally through the Corporate Professional Practice and Quality team, with guideline implementation supported at the site level.

Our participating communities include:

  • Yee Hong Centre – Scarborough McNicoll
  • Yee Hong Centre – Scarborough Finch
  • Yee Hong Centre – Markham
  • Yee Hong Centre – Mississauga
  • Yee Hong Centre – Ng Siu Chan Care Centre (integration into the BPSO implementation structure in Spring 2027)

Each community contributes to the BPSO journey through interdisciplinary collaboration, frontline engagement, implementation of selected guidelines, and ongoing quality-improvement activities.

While BPSO governance, public communications, reporting coordination, and knowledge-sharing activities are managed corporately, local teams play a central role in adapting and sustaining best practices within their communities.

Yee Hong recognizes the contributions of staff and leadership across all participating sites in advancing a shared culture of evidence-based care.

Building Capacity Through Best Practice Champions

Best Practice Champions are essential to the successful implementation and sustainability of evidence-based practices.

Yee Hong’s BPSO initiative supports the development of interdisciplinary Champions who serve as advocates, educators, collaborators, and change agents within their teams.

Champions help translate best-practice recommendations into everyday care by supporting staff engagement, sharing knowledge, identifying opportunities for improvement, and promoting collaboration.

Through education, mentorship, and ongoing learning opportunities, Yee Hong aims to build leadership capacity across its workforce and empower staff to contribute meaningfully to quality-improvement initiatives.

This approach supports a culture of shared responsibility, innovation, and continuous professional development.
